Complete the forms to be eligible to play soccer in the United States.

Get FYSA and US Club Soccer passcard for the player.

1. Log in to your GotSport account. https://system.gotsport.com/users/login

Username: your email Password: your password

If you don’t have a GotSport Account just open your invitation e-mail and Click on ACCEPT INVITATION.

2. Click the “Family” icon, then select your child’s US Club Soccer or USYS account.

3. Click “Forms” and complete the required forms.

4. Upload a photo of your child.

5. Go to Documents and then upload a Birth Certificate or Passport of your child. Do it to both US Club Soccer and USYS.
