WAIVERS
Attached are waivers that need to be signed by anyone who will be on the field participating in the event on Saturday. Coaches need to sign the waiver titled Adult, and parents/ guardians need to sign the waiver titled Minor for their player that is participating.
Please have coaches and parents print this out and sign it as it will be collected before the athlete can participate.
